REQUEST MEDICAL RECORDS
Please complete the release form and email it to records@capitolpain.com, fax it to 855-757-0638, or drop it off in person at one of our clinics. We’ll process your request within 10 business days.
SEND MEDICAL RECORDS
If you or your current provider needs to send outside medical records, please fax CPI at 855-757-0638.
